
Following a successful clinical trial and multisite pilot, Desert Oasis Healthcare, a medical group affiliated with Heritage Provider Network (HPN) and Rimidi announced a strategic partnership to deliver a disease management platform powered by clinical decision support algorithms for both diabetes care and congestive heart failure across the greater Palm Springs, California area.
Atlanta-based Rimidi is a digital health startup in the Advanced Technology Development Center’s ATDC Signature portfolio of companies. Desert Oasis Healthcare is an integrated healthcare delivery system. The companies have partnered to offer enhanced care for patients with diabetes and congestive heart failure using Rimidi’s software platform. Under the agreement, Heritage Provider Network will offer Rimidi | Diabetes+Me® and Rimidi | CHF to affiliates across its broad network.

The Diabetes+Me® program is now standard of care for patients with diabetes at HPN in Coachella Valley who can access Rimidi’s application and share data from connected glucose meters and biometric devices. In addition to tracking their health data, patients can use the app to access their care plan and diabetes self-management education online and on mobile devices.
Care teams including nurses, pharmacists and physicians use Rimidi’s unique clinical decision support algorithms to make targeted treatment decisions about patient care and to share that information with the patient via the secure software platform. The platform enables clinicians to make individualized care decisions as supported by clinical evidence and practice guidelines.
Rimidi has received funding from SOURCE, a partnership between the Hitachi Foundation, Investor’s Circle, and Village Capital, as well at the Sorenson Foundation. The company also received funding from the JumpFund, whose mission is to invest women’s capital in female-led companies with growth potential, and from Cox Enterprises.
